Workers → gpt-5.6-sol at high effort; assistant → gpt-5.6-terra at medium (up from low).

Set in all three places a role can resolve a model, which previously disagreed: the CODEX_HOME config.toml default (was hardcoded and drifting), the per-role KEYSTORE_CODEX_MODEL* env, and the organization.toml tiers the dispatcher falls back to in pickRoleCodexModel.

Both models verified against the live API before landing.
